BAR CARD APPLICATION

PLEASE NOTE:  Our office has relocated to 220 W. Willow St., Bldg. C, Ste. 102,  Lafayette, LA  70501.  We are in the center of the Clifton Chenier Center complex, in the same building as the Library and the Town Hall Auditorium.  

Bar Cards Processing Hours:

Monday – Friday
8:00 a.m. – 12:30 p.m. ONLY

MUST PRESENT WITH A VALID PICTURED ID AND SOCIAL SECURITY CARD TO APPLY
